• valid passport for at least 6 months beyond stay, and with at least one blank visa page
• completed and signed Belarus visa application. All questions in the Belarus Visa application form must be answered. Failure to complete all required fields in the form will delay the processing of your Belarus Visa.
• one recent picture (passport size)
• check or money order in the amount specified below, depending on Belarus Visa type plus shipping expenses
•  ( $15.00 priority service or $ 25.00 overnight service )
